- The character of the Doctor was created by Rick Berman, Brannon Braga and Jeri Taylor based on concepts originally developed by Gene Roddenberry.
- Original promotional material identified the EMH Mark-I as Doctor Zimmerman. This was changed before the series first aired and the character was known only as The Doctor. The Zimmerman name was later attributed to Lewis Zimmerman, creator of the EMH as first mentioned in the season one episode "Phage".
- The Doctor is the second non-sentient being to hold a senior officer's rank in the Star Trek mythos. The first was the android Data from Star Trek: The Next Generation.
